Surgery Techniques
Types of Blepharoplasty
The eye area and associated conditions that makes someone consider Blepharoplasty is quite complex, and depends on a number of factors. The technique that Dr. Buonassisi will discuss with you depends on where exactly the condition lies, and whether it’s the upper bleph, the lower bleph, and in some cases both.
Upper Blepharoplasty
Optimal for tightening upper eyelid skin and muscles, the reduction of eyelid puffiness, and to help make the eyes appear bigger, brighter, rested, and refreshed. A precise incision is made in the upper eyelid, excess skin and underlying fat is then removed, and the resulting scar will be hidden in the natural crease of the eyelid once healed.
Internal Lower Blepharoplasty
Best suited for those who have noticeable puffy lower eyelids due to herniated bulges of fat (typically hereditary), and do not have loose skin that needs to be removed. A very small incision inside the lower eyelid is made, where it cannot be seen. The tissues are gently separated to expose the lower eyelid fat that often bulges forward. The fat is carefully removed in order to reduce the puffiness of the lower eyelid.
External Lower Blepharoplasty
Removes excess and aged skin on the lower eyelid, and helps to improve a tired looking appearance. The skin and underlying muscle is gently elevated to expose the fat that often bulges forward from the lower eye. Once the fat is removed and reshaped, the excess skin is trimmed and the underlying muscle is suspended upward and outward to support the tissues for long-term tightening of the lower lid.
Sliver Blepharoplasty
Silver blepharoplasty surgery is a minimally invasive cosmetic procedure requiring less downtime that a normal Blepharoplasty. It rejuvenates the eyelids by removing excess skin and fat, resulting in a more youthful and refreshed appearance, and enhanced self-confidence for the right candidate.

Frequently Asked Questions
Who is a candidate for blepharoplasty?
Patients who are excellent candidates for these procedures have these concerns as they age or as a result of hereditary factors:
- Droopy or hooded eyelids
- Sagging eyelid skin
- Tired, puffy eyes and under-eye bags
- People who do not have a natural eyelid crease
To determine if you are a candidate for blepharoplasty, our patient care managers are available to help discuss the surgery in more detail and conduct a photo review.
How is blepharoplasty performed?
Blepharoplasty (depending on the specific technique) typically takes around 1 hour and is performed under local anesthesia. Blepharoplasty requires removing excess skin and fat to help rejuvenate your appearance, and bring focus back to your beautiful eyes. Dr. Buonassisi will begin the surgery by carefully measuring the skin that needs to be removed. Then, precise incisions are made to remove the skin in such a way that the stitching is hidden within natural creases of the skin. Once the sufficient amount of skin, fat, and/or muscle have been removed and the desired result has been achieved, the stitches are placed to complete the surgery.
As each patient is different, and there are a number of different blepharoplasty techniques to address every concern, a more comprehensive discussion about the surgery will be discussed in your pre-consultation with a patient care manager, followed by your consultation with Dr. Buonassisi.
How long does it take to recover from blepharoplasty surgery?
We suggest that patients take about a week off of work, and about 2 weeks off from strenuous activities and exercise. Most patients are comfortable being back in the public eye after a couple of weeks, but full recovery takes several months. There are no visible scars from the incisions. You can expect to enjoy your results for at least 10 to 15 years, but as you age, your skin will begin to naturally droop over time.

What is blepharoplasty surgery?
Blepharoplasty, also known as eyelid surgery, is a procedure designed to improve the appearance of the eyelids. It can be performed on the upper eyelids, lower eyelids, or both. The surgery removes excess skin, fat, and muscle, which can create a more youthful and refreshed appearance.
What are the benefits of blepharoplasty?
The main benefits of blepharoplasty include a more youthful and rested appearance, removal of excess skin or fat that may cause sagging, and improved vision in some cases where drooping eyelids obstruct the line of sight. The procedure can help individuals look more awake and refreshed.
Will I have visible scars after blepharoplasty?
Incisions made during blepharoplasty are typically very small and are placed along the natural creases of the eyelids to minimize visible scarring. When working with a skilled and experienced surgeon, most scars are well-hidden in the eyelid folds and fade significantly over time. Proper aftercare can also help reduce scarring.
How should I prepare for blepharoplasty surgery?
Prior to surgery, you’ll have a consultation with your surgeon to discuss your goals and any medical conditions. You may be asked to stop taking certain medications, avoid smoking, and refrain from eating or drinking after midnight on the night before surgery. The surgeon will also provide specific instructions for post-operative care that will be reviewed with you prior to your surgery day.
What can I expect during the blepharoplasty procedure?
Blepharoplasty is typically done under local anesthesia with sedation, though general anesthesia may be used for more extensive procedures. The surgeon will make small incisions in the natural creases of the eyelids and remove excess skin, fat, or muscle. The procedure usually takes between 1 to 2 hours, depending on whether both the upper and lower eyelids are being treated.
