How Do I Receive and Complete My Rhinoplasty Consent Forms?
Once you have scheduled your surgery, I will manage all the details of booking your surgery with the surgery center and making sure you are well prepared for surgery. I will provide several documents that need to be signed or actioned by you. These include:
1) consent forms
2) pre and post operative instructions
3) lab requisitions
4) history and physical
5) prescriptions.
Consent forms are essential - they ensure that you understand the risks of surgery that were discussed with you during your pre operative consultation with Dr. Buonassisi. Consent forms are required by the College of Physicians and Surgeons of BC when your surgery is booked with the surgery center.
